Very Easy to use Autograph Log sheet

Here's a Very easy to use Autograph log sheet that I made. It took me about an hour to finally get it the way I
Liked it. Like I said It's very Easy to use.

Let me know what you think!! You can print off as many copies as you like, It's meant to fit a 8 x 11 piece of
paper.

Hey Ryan.....I like the form a lot! That is great for keeping a hard copy record. Sometimes paper is easier than computers.

I keep track of my requests in an old version of Works Database. It's great because you can check off what you've received...and it counts it and all for you. Bad thing is I broke down the address by city/state/zip/country.....and 200+ requests later I've realized it would have been easier to just have blank lines. The other downside is I don't think I'll be able to convert it to a newer Works or Office format.

Good job on that form.
